Tax
Organizer for Individuals |
Our Tax Organizer is designed to help you maximize your deductions
and minimize your problems in preparing and filing your tax return.
The organizer is comprised of seven parts, each developed to assist
clients in collecting relevant tax information needed to properly
prepare your tax return.
Access any of the seven parts by clicking the desired part number
below and print if needed. Drop-off or mail in the completed pages
along with any required documentation (listed in part 7) to our
office for immediate processing.
• Part 1:
REQUIRED FOR ALL RETURNS – includes
instructions, taxpayer information, 2009 questionnaire, miscellaneous income, and refund direct deposit
information.
• Part 2: Interest, dividends and stock sales.
• Part 3:
Needed if itemizing your deductions –
Medical expenses, deductible taxes, interest deductions, and child
care expenses.
• Part 4:
Needed if itemizing your deductions –
Charitable contributions, education expenses, miscellaneous
deductions, and casualty losses.
• Part 5: Travel
expenses, away-from-home expenses, moving expenses, home sale &
purchase, and office-in-the-home.
• Part 6: Rental &
business income.
• Part 7: Checklist of documents and records needed.

Some items to consider when mailing tax return data to Clark
Financial Services:
- Send copies of your
required documentation (i.e., Forms W-2,
1099s, brokerage statements, etc. Please do not send the original
documents via mail!
- Use an overnight service (i.e., FedEx or UPS) or utilize the
US Postal Services priority mail with confirmation tracking.
- If you email our office (info@clarkfs.com) advising us that
you have sent your tax materials via mail, we will notify you of
their receipt.
- We will call or email you if we have any questions, comments
or suggestions regarding your tax materials. Also, we will provide
an estimated completion date for your return.
- If you utilize the mail in service, please send your
information prior to March 21st to ensure your data is received
and processed in a timely manner prior to the April 15th filing
deadline.
